Summary & Objective

The Admissions Registered Nurse is responsible for evaluating referrals and coordinating patient/family needs for admission and support all sales activities within the territory assigned.

Knowledge & Experience Requirements

  • Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred.
  • Clear and Active Registered Nurse license to practice in the state of Florida.
  • 1-2 years of nursing experience preferred. Hospice nursing experience preferred.
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
  • Must have knowledge of computer office/clinical software.
  • Must be able to read, write and understand the English language.

Essential Functions

  • Evaluates assigned referrals for appropriateness for admission and verifies terminal diagnosis and prognosis.
  • Communicate program explanation including risks and benefits of hospice care.
  • Obtain written consent from patient, family or patient assignee as appropriate.
  • Performs the patient assessment and physical on admission to determine the current health status of the patient and establishes the written plan of care with the IDT.
  • Evaluate and insure accuracy in the preparation of documentation.
  • Provides accurate and appropriate documentation of patient/family services.
  • Adjusts work assignments and schedule in response to changing patient and/or organizational needs.
  • Contacts patient/family within twenty-four (24) hours to set up appointment to explain hospice services.
  • Communicates to patient's attending physician, referral source and Catholic Hospice office staff patient's/family's decision regarding hospice services.
  • Visits referral sources to educate about Hospice services. Provide outreach to territory sales plan and coordinate with Account Executive.

Other Duties

  • Ensure effective communication with Interdisciplinary Teams.
  • Participates in the admission team responsibility of providing seven (7) days week coverage; including evenings and holidays.
  • Recognizes, reinforces and enhances the role of all IDT members in Hospice services provided.
  • Discuss service recovery options with your manager, so that if or when a service recovery need is identified, you have the confidence to “take action” immediately to begin the process to improve the issue.
  • Educates the community regarding hospice philosophy and concept through a case by case consultation with person in the community or educational in-service or seminar.
  • Participates in providing education to team members regarding nursing aspects of patient/family care on admission to the program.
  • Participates in orientation from initial classroom to field integration.
  • Demonstrates knowledge of Catholic Hospice, Inc., policies and procedures and pertinent State and Federal Rules and Regulations.
  • Ability to travel to any location throughout South Florida as needed and expand territory as business needs dictate.
  • Participate with on-call rotations.
  • Maintain your required licenses, certifications and mandatory skill updates.
  • Comply with all policies, local, state and federal laws and regulations.
  • Provide other duties of healthcare team member
  • Perform other duties as assigned

Supervisory Responsibility

  • May serve as an interim department leader depending on need

Physical Requirements

  • Must be able to lift and/or move up to 50 pounds and push/pull up to 250+ pounds, walk, climb stair or ladders, stand on feet for extended periods of time, etc.
